On your first visit with Ann Arbor Saline Dermatology you will meet our doctor for an interview and examination. BEFORE YOUR VISIT
OUR WAY OF MAKING THINGS A LITTLE BIT EASIER FOR YOU
Complete patient paperwork on our portal to minimize your waiting room wait times and maximize your safety with social distancing. You should have received an email from noreply@ezderm.com to sign up for our patient portal. Verify your email and then complete your intake forms.
Sign our consent forms on the portal (under "appointments" on the top right).
Bring any records you have from a previous dermatologist (biopsy results, treatments, notes, etc). Even better, have your previous dermatologist fax them to our office ahead of time so they can be reviewed before your appointment.
Bring a list of any medications you are currently taking.
If you have insurance, bring your insurance card and any other insurance information you have available.
Patients under the age of 18 must be accompanied by a parent or legal guardian to receive treatment, otherwise they will be rescheduled.
