Insurance and Payment: List

INSURANCE

We currently accept most insurances and new plans are being added; please contact our office as well as your insurance company to verify whether or not we accept your particular insurance.  Verifying details of your insurance coverage is ultimately your responsibility. You should also call the number on your insurance card to verify that Ann Arbor Saline Dermatology is a covered provider.

OUT OF NETWORK PLANS AND MEDICAID

There are some plans we are unable to accept. Unfortunately, the extraordinarily high administrative and regulatory burden of these programs make it impossible for us to serve these patients.  In addition, because we are privately owned and small, we have been unable to secure fair contracts with some insurance companies and Medicaid.

For our patients who are out of network or uninsured, we offer dermatology services billed directly to the patient, instead of your insurance.  Payment is due at the time of service.

REFERRALS

Some insurance plans require you a referral from your primary care physician. Since this is the result of your contract with your insurance company, if a visit is denied because a referral has not been obtained, you will be responsible for the cost of the visit.

DEDUCTIBLES

Please be aware of your specific plan and whether or not you have a high deductible. Some insurance plans even have a separate deductible amount for office visits, procedures, and laboratory or pathology tests. If you have a high deductible that has not been met for the calendar year, you will need to pay the charged amount out of pocket. We do our very best to provide an estimate for the potential costs, but some insurance companies are not transparent about their fee schedules for office visits or procedures.

CREDIT CARD ON FILE

Our office does maintain a Credit Card on File policy. After your visit, your insurance company will send you an EOB (explanation of benefits) that tells you the balance they have determined is owed to Ann Arbor Saline Dermatology based on your contract with your insurance company. Your outstanding balance will be charged to a valid credit, debit, or HSA card that you are required to leave on file with our office. Co-payments can be paid at the time of your visit with the payment method of your choice (cash, credit card) or with the card on file. In no circumstances can we waive co-payments. Services that insurance does not cover, such as cosmetic procedures like laser treatments, Botox, fillers, and chemical peels, require full payment at the time of service.
